Copy of CDL
To be provided for all qualified Company Drivers using dispatch services.
Signed W-9
To include your Federal Tax ID Number.
Proof of Insurance
$100,000 in Cargo Insurance
$1,000,000 in BIPD Bodily Injury/Personal Damage Liability Insurance
Carrier/Dispatch Agreement
Standard agreement between Carrier and Dispatch.
